2004 Partners in Transformation Awards Program (March 23, 2005)

Eligibility Requirements

To enter the contest, your organization must be:

1. A faith-based nonprofit organization or house of worship/congregation with an annual budget of less than $5 million (cash plus in-kind donations).

2. Operating a social service program in AK, AL, AR, FL, ID, MI, NJ, OH, OK, or TX in collaboration with one or more organizations OUTSIDE the faith sector (e.g., schools, police, government agency, Chamber of Commerce, secular foundation or nonprofit, etc.) Examples of social service programs include (but are not limited to): educational programs, mentoring programs, job training and placement, substance abuse treatment, homeless services, affordable housing, criminal justice programs, teen pregnancy prevention efforts, etc. FASTEN reserves the right to disqualify an applicant that nominates a program not considered by FASTEN to be a “social service” program.

Complete the cover sheet and sign the waiver indicating that FASTEN and the Points of Light Foundation are free to publish information about the nominated program and its sponsors and to include contest entrants in the FASTEN Directory, a national database of faith-based organizations, intermediary organizations, public agencies, and private foundations hosted on the FASTEN Web site for purposes of cross-sector networking. 4. Submit complete applications to FASTEN so that they are received by mail, overnight delivery, or electronically by midnight on April 1, 2005.

How the Contest Works

1. Applicants will complete Round One applications (the application can be downloaded at the link below and mailed into FASTEN or completed on-line).

2. A panel of judges will review and score all applications and select 5-10 semi-finalists from each of the ten states. Semi-finalists will be notified of their status by May 2, 2005.

3. Semi-finalists will each be awarded $100. This award will be presented to the FBO nominating the program.

4. Semi-finalists will complete a follow-up questionnaire providing additional information about their collaborative program. This will include providing the name of and contact information for an independent reference who can comment on the program (this must be a person with no formal affiliation to the program – staff members, volunteers, Board members cannot serve as references.) The follow-up questionnaire must be received by FASTEN by midnight on June 5, 2005.

5. A panel of judges will review the follow-up questionnaires and a FASTEN staff person will complete a brief telephone interview with the independent references. If needed, judges may also contact the contestants directly for additional information.

6. The ten state winners will be notified on July 1, 2005.

Awards

1. Each of the ten state winners will receive a $5000 cash award. This will be awarded to the faith-based organization (FBO) that has entered the contest and it is intended to support the collaborative social service program nominated by the FBO for the Partners in Transformation contest.

2. Each of the ten state winners will receive public recognition by their state Governor as the state winner in the 2004 Partners in Transformation contest.

3. FASTEN will publish a report highlighting information about the ten winning programs and disseminate it to private charitable foundations.

Instructions for Entering the Contest

1. Complete the cover sheet and sign the waiver.

2. Fully complete the contest application, being sure to answer every question carefully and thoroughly. Applications should be typewritten or handwritten (print) in clear, legible fashion using a black pen. Additional information may be supplied, but should not exceed two pages.

3. Submit the cover sheet and application to FASTEN electronically using the on-line application form or by mail or overnight delivery. If mailing, submit completed cover sheet and application to:

Statement
FASTEN reserves the right to disqualify applicants whom the judging committee deems ineligible for any reason. All decisions by the judging panel are final.
